We accept all insurances, including Medicaid. Financial assistance is available based on income levels for the cost of testing or if you have a balance after insurance pays. A self-pay price of $295 is available if you do not have insurance or do not wish to bill insurance.
We bill your commercial insurance or Medicaid. What you owe depends on your specific plan — your deductible, coinsurance, and whether Encompass is in-network or out-of-network for your plan. Some patients pay nothing; others may owe more, depending on plan design, coinsurance or how much of any deductible remains in your plan year.

We offer a robust Financial Assistance Program to help reduce the potential out-of-pocket costs associated with testing.
This offering applies regardless if your insurance covers part of the cost, denies the claim, or you’ve chosen to self-pay without submitting a claim to your insurance. If the amount you owe is a hardship, our Financial Assistance Program can reduce the amount you owe based on household income.
If you qualify for this program the estimated cost is between $0 and $150.
HSA / FSA eligible: Encompass qualifies for HSA and FSA funds. You can pay directly from either account or submit your receipt to your plan administrator.

What will be the cost of my test?
Your cost will vary if you are using insurance based on what your insurance covers and any copays, coinsurance or deductibles you may have. If you are uninsured or choose to not bill insurance, your payment will be $295. If this is a hardship, you may also apply for financial assistance.
What if my insurance denies the claim?
The $295 self-pay price applies automatically — you won't owe the full billed amount. We can also file an appeal on your behalf if you'd like; just let our billing team know and we'll send you a short authorization form.
Can I use my HSA or FSA?
Yes. Encompass is HSA/FSA eligible. You can pay directly from either account online, or submit your receipt to your plan administrator for reimbursement.
Is a payment plan available?
Yes — payment plans are available for the self-pay amount. Contact billing (billing@encompasstest.com) to set one up; there's no interest or fees.
What's a Good Faith Estimate, and do I get one?
If you're uninsured or choose not to use insurance, federal law (the No Surprises Act) entitles you to a Good Faith Estimate before service. If your final bill comes in $400 or more above that estimate, you can dispute it.
How do I apply for financial assistance?
Email billing@encompasstest.com or call 888-850-8812. The Financial Assistance Program is available whether your insurance is covering part of the cost, denied the claim, or you're choosing the self-pay price — we'll ask for your household size and income to confirm your tier.
